LAKE AREA WATERSPORTS PHOTO RELEASE FORM: This form needs to be completed for every minor participant.*

FLORIDA MINOR FORM: This form needs to be completed by the parental guardian for any athletes under the age of 18.*

WWA MEMBERSHIP: All surfers participating in the contest MUST be a member of the WWA. You can purchase a one-event membership HERE or you can purchase an annual membership HERE that is good for WWA events through 12/31/2020.*
